Complex odontogenic infection with focal partial thrombosis of the external jugular vein: case report

Abstract

Introduction: Odontogenic infections, especially in patients with comorbidities such as diabetes and hypertension, have a high potential for dissemination, constituting medical emergencies that require immediate interventions. Case report: A 64-year-old male patient presented with dental pain for a month, progressing to submandibular abscess and right cervical edema. He exhibited dyspnea, dysphagia, and signs of severe systemic infection. He underwent urgent surgical drainage under general anesthesia, with excision of infectious foci. During treatment, he was admitted to the ICU, receiving intravenous antibiotics and control of glucose levels and blood pressure. Laboratory tests revealed partial thrombosis in the external jugular vein, and anticoagulant therapy was started. After three days, the patient improved, was extubated, and made a full recovery in 50 days with home antibiotic therapy. Discussion: Comorbidities such as diabetes and hypertension exacerbate odontogenic infections, compromising the immune system, hindering healing, and increasing the risk of severe complications. Conclusion: This case highlights the importance of early diagnosis, immediate intervention, and intensive management for the effective treatment of severe odontogenic infections.
