Minimally invasive deep neck lift: advances and predictable outcomes

Abstract

Background: Poorly defined neck contour is a common aesthetic complaint among patients seeking facial rejuvenation. Traditional neck lift techniques are often invasive, involving extensive incisions and skin excision. Objective: To describe a systematic approach to the Minimally Invasive Deep Neck Lift (MIDNL) performed through a single submental incision without skin excision, emphasizing patient selection, step-by-step surgical technique, and aesthetic outcomes. Case Report: A detailed technical description was conducted based on clinical experience and the literature. The procedure includes precise anatomical markings, deep layer dissection, subplatysmal lipectomy, selective management of submandibular glands, partial digastric myotomy, perihyoid fasciotomy and anterior platysmaplasty, with no skin removal required. Clinical cases are also reported. Conclusion: MIDNL is a predictable and reproducible option for young and middle-aged patients with cervical volume excess and good skin quality, achieving consistent results and a discreet scar concealed in the submental region.
