Alveolar cleft reconstruction with mandibular symphysis graft: case report

Objective: To explain a successful case of alveolar cleft reconstruction using a autogenous bone graft harvested from the mandibular symphysis, in combination with biomaterials. Case Report: A patient with a unilateral cleft involving the lip and alveolus on the right side of the maxilla presented with aesthetic concerns regarding nasal appearance, as well as symptoms related to an oronasal communication. Bone graft surgery was performed using a block of autogenous bone harvested from the mandibular symphysis, supplemented with particulate graft material and covered with a bovine-derived membrane. Postoperatively, the patient developed infection and wound dehiscence, which were managed successfully with antibiotic therapy and adjunctive laser therapy. The healing process progressed well, leading to complete closure of the surgical wound. Conclusion: The technique involving an autogenous graft from the mandibular symphysis yielded favorable results, including closure of the oronasal fistula, with no evidence of flap necrosis or injury to adjacent structures. The reconstruction of the bone defect allowed for improvements in speech, chewing, and aesthetics.
