Comparison between alveolar bone grafting with autogenous bone and autogenous bone associated with PRF/PRP in patients with cleft lip and palate: a systematic review

Abstract

Objective: to compare alveolar bone grafting (EOA) using only autogenous bone (OA) and EOA with OA associated with platelet aggregate (PRF/PRP) in patients with cleft lip and palate (CLP) to evaluate the efficacy of the biomaterial association in bone neoformation. Methodology: This systematic review was performed in the Pubmed, Embase and Web of Science databases. A classification of potential risk of bias was used to evaluate the included studies. Patients, Participants: Patients with CLP. Interventions: EOA with OA only and associated with PRF/PRP. Outcome: Measurement of bone formation after EOA with and without PRF/PRP. Results: Among the 2240 literature retrieved, 15 articles were included, including 380 patients with CLP who underwent EOA (with OA only and associated with PRF/PRP). Among the included studies, 9 did not show statistically significant differences. Despite this, the use of these platelet aggregates associated with OA from intraoral regions or biomaterials showed equivalent results and can be used for reconstruction of bone defects in both unilateral and bilateral cleft lip and palate, with the advantage of increasing the volume of the material. Conclusion: The use of PRF/PRP in EOA did not show statistically significant differences when compared to the control groups, but the use of PRF/PRP associated with OA from intraoral regions or biomaterials showed similar results. They can be used for reconstruction of alveolar bone defects in both unilateral and bilateral CLP, with the advantage of increasing the volume of the material, and the use of OA from the iliac crest can be avoided.
