Delayed removal of a third molar displaced into the maxillary sinus in a pediatric patient: case report

Abstract

Objective: To report a case of a displaced upper third molar in the maxillary sinus in a pediatric patient, with delayed removal using the Caldwell-Luc approach. Case report: A 15-year-old female patient, ASA I, presenting with facial pain, headache, frequent rhinorrhea, and right nasal obstruction. A history of tooth extraction of tooth 18 at 11 years of age was reported. Cone beam computed tomography revealed a hyperdense image compatible with a tooth and total opacification of the right maxillary sinus. The initial treatment for sinusitis was antibiotic therapy and the use of a nasal decongestant. Subsequently, the surgical procedure was performed in an outpatient setting under local anesthesia using the Caldwell-Luc technique. In the postoperative period, the patient presented with mild epistaxis, with no signs of infection or complications. The follow-up radiograph after 30 days showed satisfactory healing. Conclusion: This case reinforces the importance of appropriate surgical indication in relation to age and stage of dental development in order to avoid surgical complications. In addition, it highlights the effectiveness of the Caldwell-Luc approach as a safe alternative for removing foreign bodies from the maxillary sinus.
